Hold Policy:

As stated in your membership agreement, you have the option to put your automatically recurring monthly membership on hold for any reason one (1) time per 12 month contract for up to 14 days. If you place your membership on hold and then elect to cancel your membership, you must still provide 30 days cancellation notice and pay 40% of the remaining invoices on your account, not including time on hold.

I understand that my membership will be put on hold for the length of time I specified above and that my membership will be reinstated at the end of hold period I have selected and I authorize charges to continue as scheduled at this time.

I understand in order to hold my membership, I must submit a notice 15 days in advance of my next billing date in order for my request to be effective for the following debit.

Please indicate the date range, no less than 1 month and greater than 3 months, you want your membership to be frozen. Holds for unexpected injuries or emergencies may be able to be activated on short notice. I understand I am allowed 1 (one) freeze per year and additional freezes/holds will incur a $30 activation fee.

Cancellation Policy:

All membership agreements require 30 days written notice to cancel your membership. This form will serve as your 30-day written cancellation notice. Note that if you have a scheduled renewal payment within this 30-day period, the payment will be processed as scheduled. Your membership will be cancelled at the end of your final paid month. If you have agreed to a 6 month or 12 month term membership in exchange for a discounted rate then you will be subject to 40% of remaining dues on your current agreement. All payments are non-refundable. All grandfathered membership rates will also be forfeited and returning members will be subject to current rates.

I understand a 15-day written notice is required to cancel or terminate. I understand cancellation is based upon the billing date and will not be prorated with the final cycle charged upon cancellation notice. I understand late notices will result in continued billing until cancellation terms are met.

I understand early cancellation of any membership contract: the member will pay the difference in membership rates, the amount of discount gained via the contract, for the months they were an active member.
